Well-Being Index Survey
URMC invites you to participate in our annual Well-Being Index survey that closes Wednesday, December 4th. This validated, anonymous tool provides vital de-identified data to our Office of Wellbeing to inform its work and guide future programs.
This tool is 100% anonymous. While registration is required to help you track your own wellbeing over time, access your results, and view personalized reports and resources, your information and results are private, and your individual score will not be shared with URMC, or anyone else. We will only see summary data.
Setting up an account is quick (we recommend downloading the app), and completing the Index takes less than one minute. If you do not have an account, use the invite code URMC WELLNESS after signing up for an account.
If you already have an account, log in to take the survey
