"D/C" the CC Initiative: Enhancing Effective Communication in Ambulatory Settings

We want to share with you a new initiative titled D/C the CC, with the primary objective to elevate sharing of important information, while reducing the burden of unimportant information automatically sent to our internal PCPs.

 

To address this challenge, auto-routing of outpatient visit notes to UR Medicine PCPs and URMFG will no longer occur as of June 12, 2023. Notes to EXTERNAL providers (non-employed community providers) will continue to route automatically. 

 

Specialist Best Practice Guidelines:

  • We are asking specialists to actively assess the necessity of sharing each note.
    • Only send notes to team members if there is a significant change in status and plan for your mutual patient, or if it is necessary that the recipient knows this new information at this time. 
    • Exercise discretion, sending only essential notes such as initial consults while excluding routine follow-ups with no changes. 
  • Utilize the "Follow-up" section of the Visit Navigator to communicate actions required of the recipient (see Tip Sheet for more information).
